SUMMARY:

Palliative Care is an approach centered on improving the quality of life for patients with serious illnesses and their families, focusing on pain relief, symptom management, emotional and spiritual support, decision-making, and quality of life. This study aimed to investigate imaginative processes of nursing professionals about Palliative Care, who worked at a hospital in the city of Recife-Pernambuco, Brazil. In this doctoral thesis, we propose that Zittoun and colleagues’ model of imagination as expansion of experience (e.g., Zittoun e Cerchia (2013) can be broadened when integrated to Fogel, Garvey, Hsu, e West-Stroming’s (2006) relational-historical approach, using microgenetic analysis. The method developed in this doctoral research study involved two phases: the first was dedicated to the application of an electronic survey with twelve multiple-choice questions and two open-ended questions. Fifty-seven (57) nursing professionals from 5 hospital sectors participated in this phase of the study. The second phase involved three meetings focused on an educational process entitled Cultural Circles, developed by Paulo Freire (1991). The Cultural Circles were conducted by the researcher-facilitator with a group of seven nurses from the same hospital who had participated in the previous phase. During phase 1, we identified the frequency of responses to the multiple-choice questions, the Word Cloud technique was used to analyze the responses to the two open-ended questions. As part of phase 2, a microgenetic analysis of the audiovisual and imagery records produced during each of the three meetings was conducted. Based on the responses of the 57 participants during phase 1, the cultural landscape had imaginative processes of palliative care related to end-of-life care. During phase 2, we noticed that the nurses imagined and reimagined palliative care over time, as they participated in the Cultural Circles with the researcher-animator. More specifically, during Point Zero, palliative care was exclusively related to caring of those dealing with their impending death; Level 1 was characterized by doubt and contradiction related to what they imagined palliative care was; Level 2 started to present a more ample and complex meaning of palliative care as an approach that is not exclusively for those with their impending death; and Level 3 imagined palliative care starts to co-emerge as an approach that can reconcile both end of life care with promotion of quality of life. Additionally, we propose the expansion of the theoretical model of imagination, with the newly emergent understanding of secondary ruptures, resulting in the proposal of the "Multiple Spirals of Imagination," which describes how multiple and simultaneous ruptures in imaginative processes can occur over time. We propose that the microgenetic analysis guided by the relational-historical approach, linked to the model of imaginatio/n as expansion of experience, allows for a more complex and nuanced understanding of the dynamics of imagining over time, especially in an educational and dialogical context promoted by and through the Cultural Circles. We call for future research to integrate the study of imagination with microgenesis in other life contexts. The present work insists on strengthening palliation as a care for life in the face of the still-present symbolism that links this care to death, to finitude. We hope that this study impacts managers and health care professionals for the implementation of policies for education on palliation, strengthening the importance of the technique as a legitimate and humane approach for end-of-life care and health promotion.
