Obesity is a chronic and multifactorial disease that has a chronic inflammatory state, associated with cardiometabolic disorders, in addition to muscle and skeletal changes. All patients who have severe obesity can be treated by bariatric surgery, but the effect of surgery may be different depending on the previous metabolic status of patients. Thus, the aim of this study was to evaluate the effect of bariatric surgery on musculoskeletal, cardiovascular and metabolic parameters in women with metabolically healthy obesity (MHO) and metabolically unhealthy obesity (MUO), one year after surgery. An analytical prospective cohort study was carried out, starting in March 2018 and ending in January 2021. Data were collected from patients who underwent bariatric surgery and were considered only women for this study. A total of 35 women participated in the study, who were classified into two groups: MHO (n=18) and MUO (n=17), and assessments according to reported parameters before and after one year of surgery. The results demonstrated the both groups had equivalent weight and body fat reduction. The MHO group achieved improvements in biochemical parameters, even those that were initially unchanged, in addition to improvements in autonomic function, anthropometry and body composition, proving the benefits of surgery for these patients. However, the MHO group also showed a reduction in handgrip strength and bone mineral density, highlighting the importance of careful monitoring from the preoperative period to minimize the musculoskeletal damage thar patients experience after bariatric surgery. Of the patients in the MUO group, 88,23% (n=15) no longer met the MUO criteria after one year of bariatric surgery. Despite the benefits thar both groups had, there was little statistical difference between them after follow-up. Bariatric surgery can bring benefits, in addition to weight loss, even for metabolically healthy patients, preventing changes and the transition to an unhealthy metabolically state.
